In "Phineas and Ferb Hawaiian Vacation", he was accidentally turned into a monkey, lizard, fish and a celled organism by his own De-Evolution-ator.
In "That's the Spirit", Dr. Doofenshmirtz got himself turned into a Were-Cow.
